Google Forms is an extension of Google Docs. When using Google Form you can create different forms in order to gather information. You can choose from different types of questions (multiple choice, check box, choose from a list, open ended, fill in) in order to obtain information.  You can create an online data gathering tool for any topic you want.  There are options on this site to change font, text color, and background theme as well. 

In this assignment we were to create a online data gathering tool of our choice.  The reuiqrements were to have atleast seven questions, using an aray of different question types, with atleast one being a open-ended question.  We are to design a gathering tool that we could use in our classroom that would match a content standard.  There was to be an appropriate theme, curriculum and grade level, as well as readability of the user. 

For my online data gathering tool, I chose to do a book report form.  In this form I ask my students to fill out a number of questions, using several different types of questions. Once my students have submitted their form, I can then go and look to see who has done the assignment and what they've answered. This form is very useful in that they can do this from home or in the classroom, and I will be able to see they submitions.
